How much does in-home ABA therapy cost? The cost of ABA therapy for autism in Virginia

ABA is billed by the hour, and many hours a week add up fast. But most Virginia families do not pay the full price. Here is how the money works with Medicaid, with insurance and without either.

What in-home ABA therapy costs depends mostly on how you pay. Virginia Medicaid covers approved ABA for children under 21, private insurance covers it with your usual deductible and coinsurance, and paying out of pocket means paying an hourly rate for every hour of therapy.

The short version
  • ABA is billed in small time units, for the technician's sessions, the BCBA's supervision, parent training and assessments.
  • Hours drive cost. A plan with 15 hours a week is roughly 750 hours a year.
  • Virginia's autism law lets private plans cap ABA at $35,000 a year, unless the plan chooses more.
  • Your out-of-pocket maximum often decides what you really pay in a year.

Understanding ABA therapy costs: how ABA is billed

Insurance pays for ABA in small time units, usually 15 minutes each. Different people and tasks are billed separately:

ServiceWho does itHow often
Assessment and treatment planBCBAAt the start, then at each renewal
Direct therapy sessionsBehavior technician (RBT)Most of the weekly hours
Supervision and plan changesBCBAEvery week or two
Parent and caregiver trainingBCBAOften every week or two

On an insurance plan, in-home ABA is often billed the same way as center-based ABA. The difference is that you save on driving time and gas.

Hours of ABA therapy: why they drive therapy costs

The hourly rate is only half the picture. Some children get 10 hours a week, others 25 or more. Here's how weekly hours turn into yearly hours, assuming 50 weeks of therapy:

Hours a weekHours a year (50 weeks)
10500
15750
201,000
251,250

Your child's hours come from the BCBA's assessment and your insurer's approval. Ask the BCBA to explain the number.

ABA therapy cost per hour under Virginia's $35,000 cap

Virginia's autism insurance law, Code of Virginia § 38.2-3418.17, says private plans may cap ABA coverage at $35,000 a year, unless the plan chooses to pay more. It's worth knowing before you plan your child's hours.

We did the math. Divide $35,000 by the hours in a year, and you get the most a capped plan would pay per hour of ABA before it runs out:

Hours a weekHours a year$35,000 covers up to this much per hour
10500$70.00
15750$46.67
201,000$35.00
251,250$28.00

Our calculation from the $35,000 figure in § 38.2-3418.17(K), 50 weeks a year, all ABA services counted together. Your plan's real rates and limits may differ.

What to do: ask your plan whether it uses the $35,000 cap, and if so, how much of it your child has used. If your plan is self-funded through a big employer, state caps may not apply at all. More on our insurance and Medicaid page.

Are insurance plans required to cover ABA therapy for autism?

Many do. Virginia's law requires insurance companies and HMOs that sell plans in Virginia to cover the diagnosis and treatment of autism spectrum disorder, including ABA, for people of any age. Self-funded plans through big employers may follow different rules, though many choose to cover ABA anyway.

Before your child starts, ask your insurance provider three things: whether ABA is covered for an autism diagnosis, whether it's covered in the home, and what the yearly limit is. We check all three for you as part of intake, at no charge.

The cost of ABA with Virginia Medicaid

Virginia Medicaid covers ABA for children under 21 through the EPSDT benefit when it's medically necessary and approved. The five Cardinal Care health plans each handle approvals for their own members. Your health plan can tell you if you owe anything for approved sessions.

The bigger cost on Medicaid is usually time: getting papers together and waiting for approval. We do the authorization paperwork for you.

Insurance coverage for ABA therapy, and what you pay

With private insurance, you usually pay three things in turn:

  1. The deductible: you pay the plan's rate for sessions until you reach it.
  2. Coinsurance or a copay: after the deductible, a share of each session, like 20%, or a fixed copay.
  3. Nothing more after the out-of-pocket maximum: once you hit it, the plan pays covered care in full for the rest of the plan year.

A worked example

These numbers are made up to show the math, not a quote. Say your plan's rate is $60 an hour, your deductible is $2,000, coinsurance is 20%, and your out-of-pocket maximum is $5,000. Your child has 15 hours a week, 750 hours a year, so the plan's total is $45,000.

  • You pay the first $2,000 (the deductible).
  • Then 20% of the next $43,000 would be $8,600, but the out-of-pocket maximum stops you at $3,000 more.
  • Your total for the year: $5,000, or about $417 as a monthly cost spread over 12 months. If your plan is capped at $35,000 for ABA, check what happens after that.

Because ABA uses so many hours, many families on private insurance reach their out-of-pocket maximum early in the year. Plan for it.

The cost of ABA therapy without insurance: hourly rates

Paying privately means paying the provider's hourly rate for every unit of service. Ask any provider for their private-pay rates in writing, for both technician hours and BCBA hours, before you start. Ask too whether they offer a lower-intensity plan, such as parent training with fewer direct hours.

Before paying privately, check whether your child qualifies for Virginia Medicaid or FAMIS. Children can qualify based on family income. Ask your local Department of Social Services or apply through Cover Virginia.

Is ABA therapy for autism worth the cost?

Autism is common: the Centers for Disease Control and Prevention estimated that 1 in 31 eight-year-olds had autism in 2022. ABA is widely considered the standard behavioral treatment for autism, and the American Academy of Pediatrics says autism has evidence-based interventions that may improve function. The value shows up in daily life: a child who can ask for help, use the toilet or get through a store trip. Ask your BCBA to show you progress in graphs, so you can judge the value for your family.

Questions about the cost of ABA therapy for your child

How much does ABA therapy cost per hour?

It depends on who delivers it and who pays. Insurance plans pay their own rates for technician and BCBA time. Private-pay rates vary by provider, so ask for them in writing.

Is in-home ABA more expensive than center-based ABA?

Usually not for families with insurance: plans pay for the service, not the building. Some providers charge differently for travel, so ask.

Can ABA therapy costs change over time?

Yes. Hours often go down as your child gains skills, and your deductible and out-of-pocket maximum reset each plan year.

Are there ways to pay for ABA besides insurance?

Some families use Medicaid as a second payer, health savings or flexible spending accounts, or grants from autism charities. Ask your provider which they accept.
